Amending/Extending the validity of a Passport

Effective September 26, 2005, as an additional security enhancement and fraud prevention measure, U.S. Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a  printing error, must apply for replacement passports. Please allow approximately 2 weeks for the new passport to arrive.

1. For extra pages added to current U.S. Passports you must:

  • Complete form DS-4085 -fillable or DS-4085 -non-fillable (Application for Additional Visa Pages);
  • Submit the passport to the Consulate allowing  approximately one  week for processing
  • There is no fee for this service
  • You may drop off this application at the guard building. Please click here for these instructions. To submit this application in person click here to book your appointment, then select "Passport Services other then adding pages". 

2. For name changes, descriptive information changes, or the extension of validity for limited passports or corrections of any printing errors you must provide either:

Option A.

    • Form DS-5504 - fillable or DS-5504 - non-fillable; for applications within one year of issuance of current passport (please ignore the instructions and follow the Consulate's instructions instead)
    • The passport to be replaced;
    • The documentation required for the change; (ie. marriage certificate, divorce decree). Originals will be returned.
    • There is no charge for routine processing of these changes.
    • You may drop off this application at the guard building. Please click here for these instructions. If you would like to make this application in person you must book an appointment on-line. Please click here to book your appointment.

OR

Option B.

  • Form DS-82-fillable or DS-82 - non-fillable, for applications more than one year after issuance of current passport (please ignore the instructions on this form and follow the Consulate's instructions instead)
  • The passport to be replaced;
  • Acceptable evidence of name-change, such as marriage/divorce/adoption decree; or Court order of name-change;
  • You may drop off this application at the guard building. Please ensure that the form has been signed, click here for these instructions. If you would like to make this application in personal you must book an appointment on-line. Please click here to book your appointment.

OR

Option C. 

  • Form DS-11- fillable or DS-11 - non-fillable, for changes more than one year after the issuance of the passport but are not eligible to use form DS-82. (please ignore the instructions on this form and do not signed until instructed to do so by the Consular Officer)
  • The passport to be replaced;
  • Acceptable evidence of name change, such as marriage/divorce/adoption decree; or court order of name change;
  • Acceptable identification; (eg. drivers license)
  • Meet additional requirements for children under age 16.

State department passport forms are now available in fillable and non-fillable versions. Fillable: means that the form can be filled out on-line then printed off when completed. Non-fillable: means that the form is available to print it off only then filled in either by hand. Both of these forms must then be brought to the Consulate to complete the application process.
