Name changes, corrections & extensions of validity
 

Please read: Applying for a U.S. Passport from Outside the United States.

As an additional security enhancement and fraud prevention measure the U.S. Consulate will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a  printing error, must apply for replacement passports. Please allow approximately 2-3 weeks for the new passport to arrive.

For name changes, descriptive information changes, the extension of validity for limited passports, or corrections of any printing errors you may apply for one of the following:

Option A.

  • Form DS-5504; for applications within one year of issuance of current passport (please ignore the instructions and follow the Consulate's instructions instead);
  • The passport to be replaced;
  • Acceptable evidence of name-change, such as marriage/divorce/adoption decree or court order of name-change. Originals are required but will be returned;
  • One (1) U.S. passport photo;
  • There is no charge for routine processing of these changes.

Option B. 

  • Form DS-82, for applications more than one year after issuance of current passport (please ignore the instructions on this form and follow the consulate's instructions instead);
  • The passport to be replaced;
  • Acceptable evidence of name-change, such as marriage/divorce/adoption decree or court order of name-change. Originals are required but will be returned;
  • Appropriate fees;
  • One (1) U.S. passport photo.

Option C. 

  • Form DS-11, for changes more than one year after the issuance of the passport but ineligible to use form DS-82. (Please ignore the instructions on this form and do not sign until instructed to do so by the consular officer);
  • The passport to be replaced;
  • Acceptable evidence of name change, such as marriage/divorce/adoption decree; or court order of name change. Originals are required but will be returned;
  • Acceptable identification (eg. drivers license);
  • Appropriate fees;
  • One (1) U.S. passport photo;
  • Meet additional requirements for children under age 16.
  • Once you have completed the above steps you must apply in person at the consulate. Please book an appointment on-line
