Amending/Extending the validity of a Passport
Effective September 26, 2005, as an additional security enhancement and fraud prevention measure, U.S. Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error, must apply for replacement passports. Please allow approximately 2 weeks for the new passport to arrive.

Option A.
Form
DS-5504 - fillable or
DS-5504 - non-fillable; for applications
within one year of issuance of current passport (please ignore the instructions and follow the Consulate's instructions instead)
The passport to be replaced;
The documentation required for the change; (ie. marriage certificate, divorce decree). Originals will be returned.
There is no charge for routine processing of these changes.

DS-82 - non-fillable, for applications
more than one year after issuance of current passport (please ignore the instructions on this form and follow the Consulate's instructions instead)

Acceptable evidence of name-change, such as marriage/divorce/adoption decree; or Court order of name-change;

DS-11 - non-fillable, for changes
more than one year after the issuance of the passport but are not eligible to use form DS-82. (please ignore the instructions on this form and do not signed until instructed to do so by the Consular Officer)
