First-Time Passport Applications

I. Under the age of 16 years

  • Applicants must appear in person accompanied by parents
  • Present proof of U.S. Citizenship
  • Provide evidence of I.D. (i.e. school I.D. card)
  • Provide Parental Identification
  • Present notarized Parental Permission (form DS-3053 (fillable) or DS-3053 (non-fillable) if one parent is unable to be present at the time of application
  • Complete form DS-11 (fillable), or DS-11 (non-fillable), (please disregard instructions on form and do not sign until requested to do so by the consular officer)
  • Pay a $85 application fee
  • Submit one (1) U.S. passport photo
  • If the application is approved you will receive a 5 year passport
  • Once you have completed the above steps you must now book your appointment on-line. Click here.

II. Age 16 years and Over 

  • Applicant must appear in person
  • Present proof of identity (i.e. drivers license)
  • Complete form DS-11 (fillable) or DS-11 (non-fillable) (please disregard instructions on form and do not sign until requested to do so by the consular officer)
  • Pay the $100 application fee
  • Submit one (1) U.S. passport photo
  • If the application is approved you will receive a 10 year passport
  • Once you have completed the above steps you may now book your appointment on-line. Click here.

State department passport forms are now available in fillable and non-fillable versions. Fillable: means that the form can be filled out on-line then printed off when completed. Non-fillable: means that the form must be printed off first then filled in either by hand or typed. Both of these forms must then be brought to the Consulate to complete the application process.


- Important! -
If there has been a name change, you need to submit the document which shows the proof of name change e.g. marriage certificate, divorce decree or other name change certificate. You must submit the official document, church documents are not acceptable.
