Lost or Stolen Passports

If your passport is lost or stolen, you are advised to report the loss/theft to the local Bermuda Police Station (please see the Blue pages of the Bermuda phone book) and obtain a police report for presentation to the Consular Officer. In order to replace the passport, it will be necessary for you to appear in person at the Consulate during our American Citizen Services (ACS) hours. You should submit your application for a replacement passport as soon as possible after the loss or theft is discovered.  Please allow approximately 2 weeks for the new passport to arrive. If you are a visitor to the island and have an imminent departure date, please bring your police report and all available I.D. to the Consulate Monday to Friday from 8:30 a.m. - 3:30 p.m. walk in service, except on holidays.

If you wish to replace your lost/stolen passport you will need to:

  • Completed forms, DS-64 "Statement Regarding Lost or Stolen Passport" and form  DS-11 (fillable) or DS-11 (non-fillable)
  • Provide Proof of U.S. citizenship which includes: a US birth certificate, Naturalization Certificate, Certificate of Citizenship, Consular Report of Birth Abroad, or an expired passport;
  • If applying for a child under the age of 16, you will be required to provide in addition to the above the documents listed under Parental Consent.

State department passport forms are now available in fillable and non-fillable versions. Fillable: means that the form can be filled out on-line then printed off when completed. Non-fillable: means that the form is available to print it off only then filled in either by hand. Both of these forms must then be brought to the Consulate to complete the application process.
