Non Immigrant Visa (NIV) Forms

The following is a list of forms and documents that non-immigrant visa applicants are required to complete.

When filling out the application forms, applicants should be extremely careful to answer all questions completely and accurately and to sign the back of the form. Please note, that if a question does not apply to you, please put a n/a in that space as an incomplete application will be denied. Also, please print application forms on standard white paper.

Some of the forms below are in pdf format and require Adobe Acrobat Reader to view them.

  • DS-160 - on-line NIV Application form. Must be completed by all NIV applicants and is mandatory as of June 1, 2009. This form is replacing the DS-156, DS-157 and DS-158 forms.
